Project Title: Enhancing Mental Health Crisis Intervention to Reduce 30-Day Readmission Rates

PICO or PICO(T) Question:

Those confronted with mental health issues during their acute episodes. Development of a wide-based crisis intervention strategy, which would permit the Columbia Suicide Severity Rating Scale (SSRS) to be incorporated too. A standard care that does not include an intervention during the treatment of a sickness.
The rate of 30-day readmission decreases. The commitment to achieve this amidst the time constraints. It implements the project within a constrained time outline.

Project Description:

The initiative’s objective is principally to foster further crisis intervention protocols to reduce 30-day re-confirmation rates among individuals in a mental health crisis. Current norms in crisis management presently cannot radiate an impression of being smooth, and because of this, most outcomes are low.

Patients get repeated readmissions quickly (Marafino et al., 2021). This project is expected to fill the openings identified by implementing systematic intervention approaches consistent with this patient gathering’s personality.

Intervention Details:

The intervention comprises two key components: a complete package of services in the intervention program and the C-SSRS scale integrated into the medical care practice (Husain et al., 2023). As outlined in the NURS FPX 9901 Assessment 3 Doctoral Project Implementation Plan, the intervention crisis training will include instructor and staff education on suicide and crisis de-escalation techniques, the principles of suicide risk assessment, and post-discharge follow-up strategies. This support point aims to equip healthcare specialists with the necessary abilities and resources to prevent crises and provide timely intervention for patients.

Integration of C-SSRS:

The basic C-SSRS framework plans to expedite the process of inspecting risk for suicide and alleviate the care providers’ weight of documentation. It is a tool that validates the principal causes of creative and suicidal behaviour.

It is convenient for healthcare providers since it can be utilized to identify individuals with an increased bet of self-harm or suicide (O’Rourke et al., 2019). Utilizing structured risk assessments and patient encounters will assist with updating risk stratification and give interventions more suitable to the patient’s necessities.

Expected Outcomes:

  • Reducing Mental Health Readmissions

There is a plan that will characterize the essential outcome – cutting down 30-day readmission rates of patients with mental health disorders. Through our development of a completely fledged crisis intervention program incorporating the C-SSRS into ordinary activities, the project foresees a lower frequency of crisis and severity, prompting hospital readmissions (Brown et al., 2020). Moreover included in this project is that patient outcomes have worked on the quality of care, and continuity of care has been promoted for this patient population, which is of great importance.

NURS FPX 9901 Assessment 3 Doctoral Project Implementation Plan

Component Implementation Goals/Objectives Key Action Steps Responsible Parties Data Analysis and Evaluation Methods/Metrics
Crisis Intervention Program Decrease 30-day hospital admission rates due to mental health crises by 40%. 1. Establish standardized crisis intervention protocols. Residential service leaders, mental health providers, medical officers, nurses, social workers, etc. Quantitative: Compare admission rates before and after intervention. Qualitative: Patient and staff surveys on intervention impact.
2. Train all employees (including cleaners and security staff) in suicide risk assessment and crisis de-escalation. Training coordinators and mental health educators Quantitative: Monitor employee progress in training modules.
3. Ensure follow-up and support post-discharge through a multidisciplinary team approach. CST social workers and other professionals Quantitative: Monitor patient participation in post-discharge services.
Integration of C-SSRS Enhance suicide risk prediction performance and enable early intervention for at-risk individuals. 1. Integrate C-SSRS into routine healthcare processes. IT department, clinical leadership Quantitative: Track C-SSRS usage post-patient encounters.
2. Train staff on C-SSRS usage and comprehension. Psychosocial support workers, anxiety education officers Quantitative: Evaluate staff understanding of C-SSRS administration.
Data Analysis and Evaluation Assess outcomes achieved post-intervention to measure success against goals. 1. Collect and analyze pre- and post-intervention data on readmission rates, crisis interventions, and C-SSRS administration. Data analysis team, research coordinators Quantitative: Analyze differences in outcomes pre- and post-intervention. Qualitative: Conduct thematic analysis of feedback.
2. Monitor adherence to prescribed procedures and C-SSRS guidelines. Quality assistance team, chief clinician Quantitative: Track compliance with protocols.
3. Gather feedback from patients and staff on intervention impact. Service coordinators, improvement team, quality assurance team Quantitative: Conduct satisfaction surveys. Qualitative: Use focus groups and interviews for in-depth feedback.
Logic Model Inputs: Resources, training materials, expertise for community betterment. Develop crisis intervention by-laws. Emergency mental health specialists Outputs: Trained staff, standardized practices, integrated C-SSRS usage.
Outputs: Trained employees, standard procedures, integrated C-SSRS. Establish C-SSRS in care to enhance predictive accuracy for suicide prevention. IT department, clinical leadership Short-term outcomes: Improved staff ability in handling psychiatric emergencies and suicide risk assessment.
Short-term outcomes: Enhanced staff skills in crisis management and suicide risk assessment. Conduct onboarding and training on caregiver self-care tools. Psychological training coordinator, mental health educators Medium-term outcomes: Improved accuracy in suicide risk assessments and timely interventions for critical cases.
Medium-term outcomes: Improved quality of risk assessment and interventions for at-risk patients. Analyze readmission rates, intervention acceptance, and C-SSRS metrics before and after implementation. Data analysis team Long-term outcomes: Reduction in 30-day readmission rates post-mental health crisis intervention.
