The Patient Perspective of Person-Centered Collaborative Care

The perception of the patient regarding the person-centered collaborative care is a paradigm shift from the traditional healthcare delivery models that presuppose a passive involvement of the patients as the objects of care delivery. The perspective further dwells on the values, preferences and the experience that the patient has gone through as key factors in healthcare decision-making and treatment planning. According to the assumptions of the previous studies regarding the person-centered care interventions and the theory of human caring by Watson, the patient’s view provides the necessary information regarding the real-life application and the success of collaborative care models (Ashcroft et al., 2021).

The last measure of the effectiveness of person-centered initiatives is the patient perspective, which will lead to true feedback on the quality of care, collaboration, and achievement of personal health outcomes. As demonstrated in the discussion, even though person-centered collaborative care has significant benefits in the form of better communication and empowerment, the implementation should be successful through ensuring that the concerns of the patient are addressed.

Benefits of the Person-Centered Care Approach

Person-centered care (PCC) is a modern trend in healthcare, which is based on a focus on the individual instead of only disease processes. In putting patient views, preferences, and cultural orientations into high regard, PCC develops a more responsive and equitable structure of care. The strategy can be associated with the overall objectives of quality improvement, including the improvement of communication, the increase of trust, and shared decision-making. It is always proven that PCC is associated with the increased level of satisfaction, enhanced adherence, and more sustainable health outcomes (Cengiz et al., 2022). In the sections below, the main advantages of person-centered care, such as communication, empowerment, and holistic support, are noted.

  • Enhanced Communication and Therapeutic Relationships

Patients feel more trust and confidence when providers show that they are genuinely interested in the concerns, preferences and unique circumstances of patients. Claiming improved communication leads to therapeutic relationships that become more than clinical transactions, meaning partnerships (Greene and Ramos, 2020). The patient experience shows that respectful, transparent communication is a key factor to consider in influencing the level of treatment adherence and the satisfaction with care outcomes.

  • Empowerment Through Shared Decision-Making

The collaborative models of care enable the patient to gain power by identifying knowledge about lifestyle limitations and values. Patients note that they feel valued and respected when the healthcare teams are involved as equal participants in the process of creating treatment plans and care strategies. The empowerment results in higher involvement in self-care practices and the growth of health literacy among the patients (Cengiz et al., 2022). The transformation of passive receivers into active participants is a fundamental change in the perception of the patient about the role of healing.

  • Individualized Care Addressing Holistic Needs

Person-centered approaches encompass the physical, emotional, spiritual and social needs of patients, instead of only medical diagnoses and symptoms. When care planning decisions are grounded in cultural background, family dynamics and personal circumstances, patients like to see that their situation is taken into account by healthcare teams. The patient perspective focuses on the fact that holistic care that fulfills all aspects of health results in better health and quality of life.

Challenges Encountered by Patients in Active Participation in PCC

The anxiety and uncertainty felt by patients who enter collaborative care models are associated with new responsibilities as active team members instead of passive receivers of care. Change in substituting medical orders with various processes of complex decision-making generates mental and emotional pressure on most patients (Younas et al., 2022). It takes a lot of adaptation and confidence-building assistance to learn to express personal preferences, challenge treatment options, and take part in medical dialogues (Ashcroft et al., 2021). Patients often complain about the fact that they can do little to play a significant role in professional healthcare teams, even when they do not possess any medical knowledge.

The interplay of coordinating with various healthcare providers creates significant difficulties for patients who strive to combine different viewpoints and pieces of advice in consistent care plans. Medical terms, waiting times to access different specialities, and the time-consuming nature of medical terminology tend to make patients feel personally overwhelmed and unable to process complex information to make informed decisions.

The barriers to communication between various team members can become a source of frustration and misunderstanding among patients trying to discover the purpose of interprofessional collaborations (Younas et al., 2022). Emotional pressure of playing the role of influencing the outcome of care, therefore, running serious health conditions, makes it a psychological complication of being a team member of the patients.

Suggestions for Improving PCC Collaboration

Healthcare institutions ought to expound patient orientation programs whereby people are appropriately trained to be active members of the target groups of collaborative care delivery in advance, before the treatment is administered. The communication skills training, basic health literacy education, and the clear description of the patient rights and responsibilities in the team-based care model should be the elements of the programs (Diallo et al., 2023).

The experience of care can be supported with a long-term mentorship initiative between a senior patient advocate and a care team new employee to provide continuous support and gain confidence (Diallo et al., 2023). Learning technologies offer patients culturally relevant learning resources that they can access anywhere and read.

The medical centers should implement routine and well-organized communication sessions involving patients as the main participants, whose roles and decision-making power are well defined. The electronic health record systems must have patient-generated data, preferences and feedback mechanisms that can enable real-time input in the care planning processes.

All team members, including patients, are supposed to be provided with consistent information and have a chance to express their concerns and propose changes to the treatment plans with the help of the standardized protocols of communication (Perez et al., 2021). The healthcare teams are to establish organized feedback gathering and feedback response systems that show how patient feedback has a direct impact on care decision-making and collaboration strategies in the teams.

Future Insights for Leveraging PCC

Patient-centered medical home (PCMH) frameworks are a background of supporting patients as active team members by providing coordinated and comprehensive care delivery. The theory of human caring proposed by Watson provides the crucial insights needed by health care professionals to build the genuine therapeutic relationships that respect patient dignity, values, and personal situations when working together in care (Curcio et al., 2024).

Patient satisfaction scores and care coordination effectiveness are examples of systematic evaluation metrics that can be used to measure the effectiveness of patient-centered interventions (Black et al., 2021). All the evidence-based frameworks contribute to the practical initiatives of implementing patient engagement, as well as the complexities and challenges involved in making patients participants of the healthcare team rather than passive recipients.

Conclusion

The collaborative care approach person-centered approach on the patient side, reveals a revolution in healthcare whereby the relationship between the patient and provider is changed radically through genuine partnership and shared decision-making. An interprofessional healthcare setting, including enhanced communication, empowerment, and tailored care, has had a significant positive effect on patients.

In addition, there is still a major challenge in coping with new roles and complex team dynamics among patients who have been using interprofessional healthcare settings. The experience gained following the analysis of patient experience proves the significance of proper preparation programmes, improved communication channels, and organized use of feedback that can help a patient to become an effective member of a team. The transition to the person-focused collaborative care is not merely a clinical-improvement strategy, but also a latent recognition of patients as a useful partner.

Video Blog Script

Hi! Today, I am happy to present my video blog in which I am going to recount my experience as a patient and a member of my healthcare team that became active. The interview-based conversation talks about the advantages, difficulties, and complications of person-centered care as a patient. It is my pleasure to see you here today, as I share with you how the approach has changed my healthcare experience.

  • Interviewer

It is my pleasure to see you here today. I would like to pose you some questions concerning your last healthcare experience. What recent experience in healthcare have you had at Henry Medical Center?

  • Patient

I was brought to Henry Medical Center where I had my chronic heart failure and diabetes managed. The healthcare team engaged me right at the beginning on all decisions regarding care. I met with my primary nurse and the pharmacist and talked about my treatment goals and preferences. The staff inquired about my daily routine, family duties and worries regarding my drugs. The group developed a personalized care plan that corresponded to my values and lifestyle requirements. The method made me feel that I am respected and listened to as an individual, rather than a patient with some medical conditions. The joint working process allowed me to see myself as an active participant in my healing process.

  • Interviewer

Thanks, that is a summary of what you have to say. That is quite an all-encompassing treatment of your care. And now I would like to discuss the good things of your experience. What do you consider the top three advantages of person-centered care?

  • Patient

The most significant beneficial effect was improved communication between me and health practitioners. The team members also took time to explain to me how the practitioners carry out the procedures using words that I can understand. The practitioners were attentive to my concerns and were not speaking in a rush or dismissing my questions. The openness brought about a lot of trust and I was safe to express my concerns about treatment.

The second benefit was the participation in every decision making in health care as an equal partner. The staff also considered my opinions in regard to drug schedule and time of taking. The team also assisted me to make changes on the treatment plans based on my conditions at home and family support system. Teamwork methodology helped me feel powerful and take control over my health outcomes.

The third beneficial gain was that I got the actual type of care that is exclusive based on my own needs and circumstances. My treatment was planned by the staff considering my cultural background, my financial limitations and my goals. The hospital employees were not only attentive to my physical problems, but also to my spiritual and emotional condition. The inclusive manner demonstrated that I was a significant individual.

  • Interviewer

Those are wonderful benefits. The approach had a positive impact on your experience. While the benefits sound significant, I imagine there were also some challenges. What challenges did you face as a patient joining the healthcare team?

  • Patient

To begin with, I was overwhelmed with the task of being an active team member. I used to obediently follow the instructions of doctors. There is a major confidence-building and adjustment needed to learn how to express my views and choices. The team also taught and assisted me to learn my newly assigned position.

There was a few times a misunderstanding arose when communicating with different healthcare providers on the conflicting advice. I had to know how I could communicate well with nurses, physicians, pharmacists and social workers. The different experts possessed different backgrounds and perspectives that I needed to learn and practice. Complexity required time and clear communication of all the team members.

The engagement in essential medical discussions was sometimes too much considering the technical medical terms and medical procedures at hand. I needed processing and clarification time. The team was being trained to provide information in small portions and to revise my knowledge regularly.

  • Interviewer

Thank you for being so honest about those challenges. Your insights are very valuable. Based on your experience, how could the person-centered care process be improved to enhance team collaboration?

  • Patient

Healthcare organizations should use formal patient education programs where patients are ready to be an active participant on the team. Among the programs could be communication skills training and impartation of basic health literacy. Skills and confidence are also needed by patients to engage them in collaborative decision-making efforts.

The team-based care meetings with the patient should be held regularly. The meetings will be held in a way that there will be a sort of organized agenda and communication protocols. The team should also include patients in the provision of a meeting summary and plan of action in order to maintain continuity of care.

The healthcare facilities must adopt the use of easy to use technology platforms whereby patients can access their care plans and communicate with other members of the team. Patient-generated data and preferences need to be included in electronic health records. The technological devices will be able to increase the cooperation and to maintain the active engagement of patients between the appointments.

Concluding Remarks

My experience of person-centered care altered radically my perception of healthcare and my role as a patient. The strategy made me a more active participant in my health care, instead of an inactive recipient of care. My experience of the collaborative process enhanced my adherence to treatment and my general satisfaction with the healthcare services. In my opinion, person-centered care is the future of medical care delivery. The model respects patient autonomy as well as utilizes the skills of inter professional healthcare teams. The strategy involves both the patients and the providers making a commitment to collaborate to achieve common health objectives.
