Section I: Application of the MEAL Plan
(M) Pulmonary hypertension (PH) is a serious heart problem that arises when the blood flow to the heart is disrupted because of the elevated pressure of the pulmonary artery, which blocks the flow of blood to the lungs and reduces the level of oxygen in the blood. (E) The condition occurs because the arteries in the lungs are either constricted or obstructed in nature, and the right ventricle has to work harder. There are various underlying causes of pulmonary hypertension, such as heart disease, chronic lung disease, liver disease, blood clotting, and a few genetic diseases.
The condition is present among all ages but is more prevalent in women, non-Hispanic black people, and older adults, especially those who are over the age of 75 years. (A) PH is often diagnosed late because the first symptoms, such as tiredness and difficulty breathing, are similar to other diseases. The condition cannot be cured, but the identification and prevention of the risk factors at an early stage, including blood pressure regulation and non-smoking, can prevent or delay the onset. Medication, oxygen, and diuretics are used in the treatment to alleviate the symptoms and improve the quality of life. (L) Pulmonary hypertension is a chronic, but curable condition in case it is identified early and adequately treated; therefore, the importance of prevention, early diagnosis and multi-modal treatment by a team of medical professionals.
The severe psychological issues are most of the time linked to pulmonary arterial hypertension (PAH), which significantly affects the quality of life of patients. The recent study by Olsson et al. (2021) observed the rate and occurrence of common mental disorders among PAH patients and their effects on general well-being. The authors found that more than one-third of the patients were found to have psychiatric disorders, such as adjustment disorder, depression, and panic disorder, which were strongly associated with reduced life satisfaction and functioning. These findings support the idea that the use of screening measures, including the hospital anxiety and depression scale (HADS), can be employed in identifying at-risk patients and necessitate early intervention. The paper emphasizes the fact that mental health care must be integrated into the treatment of PAH to improve the psychological and physical outcomes.
Section II: Practice Site and Problem
The practice site is a mid-sized, not-for-profit regional hospital, located in the southern United States, which serves urban and rural patient populations. Some of the services provided by the organization include acute and specialty care services, including cardiology, pulmonology, critical care, surgical services and rehabilitation (Executive Nurse, personal communication, October 10, 2025). There are different inpatient units on the site, including medical-surgical, intensive care, and telemetry, and outpatient clinics providing chronic disease management (Executive Nurse, personal communication, October 10, 2025).
The patients in the hospital are fairly diverse, as reflected by the socioeconomic and cultural diversity of the surrounding community of working-class families, retirees, and underserved patients with limited access to medical care. In the external environment, the hospital is a participant in a competitive healthcare setting that still struggles with the problems of staffing, technology integration, and reimbursement paradigm. The organization is hierarchical with a centralized leadership that is highly focused on evidence-based practice, quality improvement, and patient safety (Executive Nurse, personal communication, October 10, 2025).
Multidisciplinary rounds, shared governance councils, and performance improvement committees are some of the best ways to promote interprofessional collaboration. The patient-centered and mission-oriented organization is founded on the values of accountability, teamwork, and compassion (Executive Nurse, personal communication, October 10, 2025). Leadership supports professional development through education, mentorship, and care devotion to innovation. Nurses and allied health professionals are busy conducting research and making decisions based on data and applying technologies, such as electronic health records and clinical dashboards, to enhance outcomes (Executive Nurse, personal communication, October 10, 2025). As a rule, the site of practice creates a friendly environment, which balances clinical quality with community outreach and involvement, prioritizing collaboration and constant patient care enhancement.
Practice Problem Analysis and Significance
The problem of mental illness among PH patients has become a significantly underdiagnosed problem in the practice site. The internal quality and case management audit reports, done recently, have revealed that many PH patients present with symptoms of depression and anxiety that are rarely assessed and addressed during hospitalization or at the time of follow-up care (Executive Nurse, personal communication, October 10, 2025). According to the providers and nurses, emotional distress, hopelessness, and fatigue are regularly reported by patients, and the referral to mental health is not regular; there is no common psychosocial screening tool.
Last year, chart audits revealed that only under 20 percent of PH patients had a mental health assessment recorded, and there was evidence of chronic disease burden and negative coping mechanisms (Executive Nurse, personal communication, October 10, 2025). The lack of systematic screening is one of the critical quality care gaps because untreated mental health conditions may result in a severe disease progression, a lower tolerance to treatment, and a poor quality of life. The difference agrees with national data that point to psychiatric comorbidities as prevalent and significant in PH populations.
Research by Olsson et al. (2021) found that over a third of patients with pulmonary arterial hypertension had mental health conditions such as adjustment disorder, depression, or panic disorder, all of which greatly reduce physical and emotional well-being. Psychological evaluation is indicated by the Pulmonary Hypertension Association as well as the American Heart Association as a comprehensive PH management approach (PHA Staff, n.d.; Rajagopal et al., 2023). However, the current practice environment falls short of the standards, whereby it does not have established policies or standard screening tools, such as a HADS, whereby early detection and intervention could be established (Lloyd et al., 2023).
Earlier intervention on the condition can result in improved outcomes. Failure to have a formalized policy in the process of identifying and meeting the mental health needs of PH patients not only impacts the patients but also the family and the healthcare system. Patients who are depressed or anxious have lower adherence to complex medication regimens, leading to avoidable hospitalization and adverse functional outcomes. The families have an increased burden of emotional and caregiving, and the organization has a higher cost due to the increased stays and readmissions (Phillips et al., 2022). The evidence-based mental health screening and referral procedures can fill the gap by enhancing the holistic care delivery, addressing the Joint Commission’s patient-centered care standards, and contributing to the hospital in its effort to enhance the mental and physical outcomes of the vulnerable population.
Implications
Lack of regular screening and management of mental illnesses among pulmonary hypertension patients has far-reaching implications in most regions. One of the economic outcomes of undiagnosed depression and anxiety is an increase in readmission, longer hospitalization, and spending on health care. This safety and quality perspective is that unaddressed emotional distress may undermine medication adherence and self-management, leading to a deteriorated clinical outcome and low patient satisfaction (Zhang et al., 2022). The absence of mental health integration is a cultural sign of enduring stigma and discontinuous care models, which omit emotional well-being.
Morally, this failure to identify and provide care related to mental health undermines comprehensive, patient-centered care and goes against the principles of professional nursing, which include providing compassion and advocacy (Zhang et al., 2022). Moreover, regulatory bodies such as the Joint Commission foresee the integration of behavioral health in whole-person treatment by the organizations; hence, the shortage may also lead to compliance and policy consequences for the practice site.
