I have fulfilled 50 practicum hours in the Oregon Health and Science University (OHSU) Family Medicine Clinic in Portland, Oregon. The practicum site offered a wholesome primary care environment that accommodates a variety of patients throughout their lives. All the hours were done in an organized clinical setting under the guidance of a certified preceptor and were not compensated, independent of any job obligations.

My practicum experience has involved thorough patient assessment exercises involving the acquisition of detailed medical history, the focused and complete physical examination, as well as the interpretation of laboratory and diagnostic tests. I have worked with my preceptor in creating the differential diagnosis and evidence-based treatment plans of the patients presenting with acute and chronic diseases like asthma, diabetes, hypertension, anxiety, and depression. I also had practical experience in the field of preventive screenings, immunization reviews, and health promotion counseling based on the needs of the particular patient.

A lot of my practicum was focused on educating and counseling the patients. I gave advice on medication compliance, lifestyle change, weight management, quitting smoking, and stress management measures. The experience reinforced my skills in effective communication and developing cultural competency in providing care to patients and their autonomy and shared decision-making.

I also engaged in interdisciplinary case discussions with physicians, nurse practitioners, social workers, and behavioral health providers to provide coordinated and holistic care. Such meetings on a team-building basis enhanced my knowledge about collaboration and care transition. Also, I was involved in the discussion of existing clinical guidelines and the implementation of evidence-based practice principles in the decision-making process related to the management of patients.

The reflection conversations with my preceptor helped me to analyze clinical performance, enhance diagnostic reasoning, and recognize the opportunities for professional development. The 50-hour supervised practicum experiences at this family medicine clinic were beneficial and helped me become more competent in clinical practice, more confident, and more ready to contribute to advanced nursing practice as well as improve my assessment, diagnosis, collaboration, and patient-centered care skills.
