I have fulfilled 50 practicum hours in the Cleveland Clinic under the outpatient internal medicine and behavioral health collaborative care environment. This practicum site was an experience that gave me the chance to work with a varied adult community accompanied by acute and chronic medical and mental health issues. The hours were all done under the oversight of an approved preceptor and were not counted as part of any employment obligations.

In my practicum, I performed a thorough patient examination with a complete history, physical examination, mental health, and medication review. I also helped in the management of chronic conditions that included hypertension, type 2 diabetes, hyperlipidemia, anxiety disorders, and major depressive disorder. In collaboration with my preceptor, I was able to come up with differential diagnoses and assist in evidence-based treatment plans that accommodated the latest clinical approaches and specific patient factors.

Much of my practicum experience was related to integrating behavioral health in primary care. I also attended depression and anxiety screenings with the help of standardized instruments, evaluated adherence to medication, and counseled patients on stress management, lifestyle changes, and coping skills. This experience reinforced my skills to treat both physical and psychological aspects of health, as well as the facilitation of holistic care.

I have also worked in an interdisciplinary team that consisted of physicians, nurse practitioners, social workers, and pharmacists. My experience in team-based case discussion improved my knowledge about coordinated care planning, in particular, the work with patients with complex medical and psychosocial requirements. I got to practice in evaluating laboratory outcomes, diagnostic studies, and treatment results to assess patient progress.

During the practicum, I took opportunities to discuss clinical reasoning, assessment methods, and clinical communication skills with my preceptor to enhance my abilities. The 50 organized hours of practicum in this state-of-the-art healthcare facility enhanced my ability to competently conduct comprehensive assessments, management of chronic illnesses, behavioral health integration, and teamwork, equipping me with widened duties in advanced nursing practice.
