Capstone Project Focus

The proposal focuses on enhancing blood pressure control and medication adherence among adults with hypertension with a lifestyle modification computer-based nurse-led program focusing on diet and exercise counseling.

PICOT Question Review

For adults with hypertension (P), the outcomes of participating in a nurse-led lifestyle modification program that includes diet and exercise counseling (I), compared to usual physician-led care (C), on blood pressure control and medication adherence (O) in 6 months (T)?

Assessment Plan Overview

Assessment 1: Full project template and First proposal.

Assessment 2: Develop a clear PICOT question and rationale.

Assessment 3: Define one intervention (nurse-led lifestyle program). Assessment 4: Discuss implementation strategy.

Assessment 5: Develop an evaluation plan to assess success.

Assessment 6: Combine papers and prepare dissemination plan.

Practicum Hour Expectations

At least 40 hours by Assessment 2, 80 hours by Assessment 3, etc. Hours should be cumulative, and one sentence written should be equal to 2 hours logged. Should outline particular things (e.g., attending meetings, discussing with patients about the best care plan).

Documentation Reminders

Turn in an Excel record of hours spent with each assessment, starting with Assessment 2. Hours must be confirmed by the preceptor. Weak words, such as shadowed preceptor should be avoided.

Preceptor and Site Engagement

Practicum activities to occur will be primarily on-site under preceptor guidance; up to a fifth of the time may be remote (e.g., via Teams or Zoom). Find ways to participate in a leadership or clinical improvement meeting.
