The data about hypertension suggests that Hypertension is one of the most health issues all over the world. In the United States 47% of the adults are suffering from hypertension (Macinko et al., 2018). 

Environmental & Epidemiological Data

Hypertension affects almost half of the adult population in the United Stated and its numbers are expected to increase in the coming years (Mathai et al., 2019). The use of community based interventions is required to help curb this health issues.

Prevalent Population Health Concerns

The rate of hypertension has been increasing over the years which is a source of major concern for the health care practitioners as well as the policymakers. Hypertension shares comorbidity with a number of diseases such as diabetes, obesity, coronary heart disease, arteriolosclerosis, hyperglycemia as well as renal and lung issues. The high rate of comorbidity of hypertension makes it an urgent health care issue (Mathai et al., 2019). 

Environmental Factors

The number of cases for hypertension have been increasing especially in the last two decades. A lot of environmental factors contribute to hypertension including chronic stress, excessive alcohol and salt intake as well as physical inactivity (Chang et al., 2019).

The proposed health care plan deals with the management of hypertension since it is one of the most prevalent health care issues in the United States.

Hypertension is a health care issue which does not have cure and the patients of hypertension are required to manage their condition throughout the course of their lives. Self-management plan is one of the most effective for the management of hypertension. This policy proposes the use of Orem’s Self-Management plan in combination with telehealth tools for the management of the hypertensive symptoms (Nordyke et al., 2019). 

Potential Barriers

The use of self-management plan is most effective only if proper education is given to the patients about their condition and the associated risks. Self-management plan if not applied through proper consideration can be harmful for the health of patients as well. Moreover the proposed health promotion plan makes use of telehealth tools. If the patients do not have knowledge or resources for telehealth communication, the implementation of this plan will not be feasible. 

Measuring Outcomes

It is hard to measure the impact of any proposed plan if the outcomes are not documented. Thus it is very important that a proper plan is set in place for measuring the outcomes for the proposed interventions. 

Outcome Criteria that will be Evaluated

The proposed health promotion plan makes use of self-management plan and the use of telehealth tools. The health progress in the patients is one of the prime outcome that we need to evaluate. This can be analyzed through the blood pressure levels of the patients since it is tracked in the telehealth application. 

Communication Plan

The proposed health promotion plan can be effectively applied for the promotion of better health in the community. Telehealth tools allow for an enhanced communication between the patients and the healthcare providers. 

Community Stakeholders

The stakeholders in this health promotion plan include firstly the patients and their families. The other stakeholders include the nursing staff and medical health care professionals. The IT team who are in charge of managing telehealth system for the hospital are also a stake holder. 

Communication Strategy

The telehealth strategy has been proposed here which will rely on electronic communication between the patients and the health care providers. The community seminars and webinars will also aid in raising hypertension related awareness at the community level (Studer et al., 2019). 

Value & Relevance of Evidence

Self-management plans have proven to be one of the most effective measures for the treatment or management of chronic illnesses. Self-management plans help the patients to take back control of their health. Self-management plans have proven to reduce the incidents of learned helplessness in patients of chronic illnesses. When combined with the use of telehealth tools, self-management plans aid in improving the condition of patients.  

Relevance and Value to Community Health Concern

The use of the proposed health promotion plan will allow the patients of hypertension to experience more autonomy over their health. Reduced number of hypertension cases will also mean a large chunk of money and medical resources which are spent on the treatment of hypertension can be saved. 

Resource Alignment to Health Improvement Plan

The use of self-management plan relies on imparting education to the patients about their condition and the risks associated with the health care issue they are suffering from. For the use telehealth tools the electronic gadget and internet facility are the resources which will be required. 

Conclusion 

The prevalent health concern of hypertension has been presented here. This assignment the hypertension has been discussed as a health. The epidemiological as well as environmental data for the prevalence of hypertension has been evaluated. A heath improvement plan has been discussed alongside the communication strategies for the implementation of the health promotion plan. The need for this health promotion plan for helping decrease the prevalence of hypertension related complications has also been discussed here.
