Hello, ladies and gentlemen! My name is ________ and today I would discuss the Hypertension which is a major and common health issue in San Jose, California primarily among the adult population. High blood pressure adds to increased risks of heart disease, stroke and kidney disease, which lower the quality of life and increase the long-term healthcare bills. The urban setting in San Jose offers environments like chronic stress, inactive lifestyles and unhealthy eating, which are the greatest known causes of high blood pressure. Also, the community members do not receive preventive services regularly, and they are not educated about health. This plan will help to curb the incidence of hypertension among the community and also increase the quality of life among the community by implementing a well-structured plan that will involve community outreach, screening, lifestyle modification and support.
