Hello Good Morning Fellows, 

Nurses play an indispensable role in performing effective and culturally-sensitive communication in healthcare settings. The coordination and continuum of care is the prime responsibility of nurses in our healthcare organization (nursing home). As a trained and DNP nurse, my duty is to be cognizant with the care coordination process to ensure safety and ethics incorporation into our practices. Indeed, we deal with patients from a diverse background and ethnicity who possess different psychological, cultural, and personality traits. These considerations play a vital role in improving the patient care outcomes for nurses, especially, in the mental health context of patients to relieve them from depression and substance abuse pain effectively (Al-Damen, 2017). This means that nurses should be aware of the community-based resources and services and understand the ethical framework provided by the state and the federal government that improves their professionalism. This is the reason, in this presentation; I am focusing on explaining and analyzing with evidence to explore the process of care coordination in the context of organizational change as a result of a change activity. I will present my findings and analysis on several issues and considerations such as culturally sensitive strategies for collaborating with patients, aspects of change management in our nursing organization, explain the rationale for ethical decision making, and identify impact of potential healthcare policy provisions. So, let’s get started! 

Culturally Sensitive Strategies for Collaborating with Patients

According to Armstrong (2018) research, limited guidance is available for nurses in healthcare settings regarding culturally sensitive communications related to interactions between patients, their families, nurses, and various healthcare professionals. Exploring the concept of culturally sensitive strategies for communication with patients and their families is vital in today’s complex environments. Despite a greater focus of healthcare professionals and nurses on the improvement of patient care related to mental health issues, culturally sensitive communication remains still poorly understood in many nursing and healthcare organizations. Hence, I believe that there is a dire need for creating awareness regarding this issue to help practitioners understand the best practices while communicating with culturally diverse mental health patients and their family members. The fact remains that when nurses take into consideration the culturally sensitive communications, mentally ill patients are likely to experience a more positive relationship with their care providers and often help to grow their relationships. The evidence suggests that many doctors and nurses in the United States are ill-prepared to communicate with culturally sensitive patients and they lack education or training to improve their skills to have better care outcomes (Burns, 2018).

Moreover, when communications are culturally sensitive, both mentally sick patients and their families are less satisfied with their experiences and perceptions of nursing care. This results in the poor adherence to care procedures and often deteriorate the quality of care with unpleasant events. As cultural is defined as a set of unique ideas, beliefs, and norms passed down from one generation to another, the importance of understanding patients’ culture is vital for clinicians and healthcare professionals. For instance, sensitive and open interaction revolves around active listening of patients’ needs and issues according to their cultural beliefs to build trust and respect. Therefore, nurses should remain respectful and provide supportive interactions to patients and their families to promote continuity of care (Burns, 2018). Where language distances exist, the use of a professional interpreter is the best practice that our organization follows. Developing a trusted relationship with patients’ family members starts with respecting their religion and culture to improve coordination of care. 

Aspects of Change Management

According to the AMA Journal of Ethics, the United States have the best and most expensive healthcare system in the world; despite having great emphasis on healthcare, the country ranks 26th in terms of life expectancy (Butts, 2019). This means improving patients’ quality of life is vital to improve their life expectancy. This reason require nurses and professionals to provide a holistic care to mentally sick patients and take better care sung the best practices and change management systems. This means that in complex situations, both nurses and patients should involve to ensure optimal care. Since professionals provide scientific expertise and information to propose the best interventions, patients’ families bring personal information to help practitioners select the best care options. Therefore, the implementation of patient-centered care can be difficult without considering problems affecting their care.

The evidence shows that informed and engaged patients are curial to have better perspectives on their own health. Most patients are not often engaged properly; hence, moving the vision of the healthcare service to patient’s centered needs is vital to bring positive change. Many researchers suggest strong focus on patients’. This means that lack of focus on patient communication is evident to make a savvy medical decision (Comfort, 2020). Moreover, some nurses explain the benefits of interventions without discussing their risks and forget to ask patients about their preferences. This shows a lack of focus on patients’ needs and preferences in care settings. Some patients are abruptly discharged from hospitals and get too much information that is hard to implement at the time of discharge. This shows that communication strategies between practitioners and patients are not effective that lack critical information at the time of discharge (Cruz Rivera, 2017). Therefore, patients can gain more satisfaction when their direct and indirect needs are met. This involves improving their level of information, discussing financial limitations, traveling issues, and their preferences. These strategies can make patients feel more respected and express their gratitude. 

Rationale for Ethical Decision Making in Coordinated Care Plans 

The evidence shows that healthcare professionals must consider several forces before making ethical decisions related to coordinated care plans. Such forces can include comprehension and knowledge of underplaying ethical rules to develop better relationships with patients. It also includes gaining more skills and knowledge related to customers’ needs and preferences to improve the care outcomes for mentally ill patients. However, many scholars suggest that ethical decision-making is the most important force to improve the outcomes of coordinated care (El Miedany, 2019). For example, nurses have to abide by the state and federal laws before sharing patients’ information with different third parties that requires them to follow professional ethics. The decisions made related to sharing patients’ information should be ethical; the ethical approach followed by coordinated care professionals must be supported by the standards and regulations within a related jurisdiction.

Moreover, the evidence also shows that decision-making should also be guided by ethical factors and regulations. Such factors can include incorporating empathy and utilitarianism (Kaufman, 2019). Deontology is another vital concept that nurses can apply for better coordinated care plans; it implies that nurses should only make ethical decisions while making decisions in their settings. Similarly, the concept of utilitarianism suggests that a coordinated care provider will always base their decisions after knowing the consequences of their actions. The utilitarianism concept reveals that decision-making is based on fairness regardless of its outcome. Therefore, the rationale for coordinated care plans state that nurses should identify aspects of change management that impact patients’ experience vital for their satisfaction. Hence, all the coordinated care decisions should be based on nurses’ code of ethics (Kavalieratos, 2017).

Impact of Health Care Policy Provisions on Outcomes And Patient experiences

Now, I will discuss the impact of healthcare policies on patient experiences and care outcomes in nursing settings. I highly believe that health insurance is one important aspect that is responsible for paying the high health-related expenses to help clients. There are several ways or patients to pay the health insurance such as using government initiatives or pay the health insurance personally. However, from one patient to another, the provisions of health insurance are different to offer them different experiences. Not all the patients have great skills to communicate with health insurance providers; moreover, some patients may have negative feedback related to insurance health provisions. This shows that many insurance companies in America fail to show credibility when it comes to fulfilling their mandate for paying clients’ medical fees. This creates a negative feedback or impression of a particular insurance organization (Palmer, 2019).

More research emphasize shift towards value-based and patient-centered methods of providing coordinated care in our nursing home. However, some facts are hard to deal with such as reducing the noise level throughout the day. This can significantly impact patients’ satisfactions scores due to beeping of machines and traffic noises outside. However, we can take small steps to improve patients’ satisfaction levels by providing our nurses and the relevant staff with the relevant training to face complex scenarios (Pest, 2020). Moreover, there is a need to implement patient feedback surveys to help share their experiences. Finally, timely communication from nurses and physicians can help patients to feel safe and not feel like being left out in the dark. Such patient data can help the business to improve patients’ experiences.

Nurse’s Role in Providing Engaging Presentations 

 The role of effective presentation and communication skills for an advanced practitioner nurse is vital. Professional speaking and presentation are a vital role we have to play on a day-to-day basis. Improving my own communication and presentation skills can help to climb the career ladder easily and can boost my own self-confidence to perform activities diligently (Peter, 2018). Significant evidence published in the recent years suggests that presentation skills and training for nurses allow them to articulate their thoughts effectively by using the body language. This improves their professional image in patients’ eyes. I embrace the use of audio-visual aids and technologies and strongly recommend nurses to use creative techniques for finding answers to key questions related to improving coordinated care services.
