Informatics and Nursing-Sensitive Quality Indicator

Hello My name is Teletha, and I am here to discuss Informatics and Nursing-Sensitive Quality Indicator. Nurse-sensitive Quality Indicators (NSQIs) are the set of rules or criteria that are used to evaluate the patient services and positive health consequences, which are done by nurses and these indicators will be implemented in their practice directly and their impact will be assessed. 

First of all, I will discuss NDNQI. The National Database of Nursing Quality Indicators (NDNQI®) is the first US national nursing database that reports structural, procedure, and quality indicators of outcome to assess nursing services at the basic level on a quarterly and annual basis. It is a voluntarily maintained collection of unit-specific quality indicators connected directly to nursing care. These figures are significant since they show the relationship across nurse-to-patient ratios, patient management, and quality output (Spagnuolo et al., 2021).

What are nursing-sensitive quality indicators?

Now I will discuss nurse-sensitive indicators.  Nurse-sensitive indicators have been used to assess nursing performance. These are described as “those outcomes that are pertinent, predicated upon nurses’ scope and sphere of competence, and as such, there is verifiable research relating nursing contributions and intervention to the output for patients. Nursing Sensitive Indicators are thought to represent the structure, method, and results of nursing care. Nursing staff availability, level of skills, and current nursing education and accreditation standards are all structural indicators. Workflow indicators are used to evaluate health assessment and clinical intervention. Job satisfaction in healthcare is occasionally regarded as a process indicator (Spagnuolo et al., 2021).

Methods of patient evaluation and nursing interventions are measured using process indicators. Employee satisfaction in nursing is often seen as a process indicator. Outcome indicators are nursing-sensitive health consequences that become reliant on the extent or quality of patient care. Nursing personnel and patient acuity, for example, would be classified as structure. Pain evaluation and intervention would be a process indicator, with the outcome being a reassessment of pain management. The two cases are cataracts and Pressure Ulcers. Inpatient readmission and cardiac arrest are some other health outcomes which are linked to some other medical aspects and therefore not to be mixed with nursing sensitive parameters. In 1999, Ten Nursing sensitive parameters were proposed in the healthcare setting for acute patients by The American Nurses Association.

ANA then added additional 11 points which were only added because they were appropriate for those institutions which are not acute in nature and are community based. Since then, the list of these points is updated and revised numerous times according to requirements and new updates added many new indicators 

Nursing-sensitive performance measures are an essential tool in creating evidence-based clinical guidelines. However, monitoring these markers is not only excellent science; it is also an ethical obligation. Nursing’s core guidelines suggest that nursing is essential in evaluating, analyzing, and improving the quality of professional nursing.

Selected Quality Indicator for the tutorial?

I choose to focus on the structural indicators of current nursing availability, competencies, and degree of education and certification. 

Importance of my chosen quality indicator

This parameter is essential to measure as it underlines the importance of nursing in just about any healthcare setting. This measure is closely related to nurses’ performance and involvement in the healthcare system, as well as favorable patient outcomes.  

Impact of Structural indicators on quality of care and patient safety.

According to a study published by the Journal of Intensive Care, increased patient – to – nurse ratios found to be correlated with a decreased likelihood of mortality.  Almost 850 patients participated in the study and it showed that mortality rate was increased up to 95% (Zhang et al., 2021). According to the Committee, nurses who are in charge of too many patients risk becoming stressed out and making medical errors as a result of a busy patient panel. According to a 2018 poll commissioned by the Massachusetts Nurses Association, over three-quarters of registered nurses think they are allocated too many patients at once. First and foremost, nurse staffing ratios and patient maximums are concerns about care quality. Advocates for mandated ratios argue that when nurses take on too many patients, patient safety and treatment quality deteriorate.

The skills and educational qualifications of nurses have an influence on how well they manage the individuals. Nurses are responsible for caring for patients in hospitals, administering medication to them, and monitoring their reaction to the treatment. As a result, they are an essential element of the hospital’s health system, and for the inpatients, they become as much a part of them as the physicians do. Not only for hospitalized patients but also outpatients, because the nurses are the first medical contact they have in any facility before being directed to the clinicians or physicians. As a result, nurses are an essential component of healthcare institutions. They determine the level of treatment offered to patients (Zhang et al., 2021). 

Why do new nurses need to be familiar with this particular quality indicator when providing patient care?

When delivering patient care, new nurses must be aware of structural quality indicators to appreciate their importance in health care practice. New nurses must be reminded of the importance of their abilities as well as the need of obtaining the appropriate certification, as they cannot practice as nurses until they are certified. Nurses’ work is critical in health care, and as a result, nurses must possess the appropriate abilities. EMR nurses, for example, must have abilities related to emergency services; neonatal nurses must have credentials and accreditation to care for newborns, and so on.  The quality of nurses at a hospital determines the level of care offered to patients and their overall happiness.

Collection and Distribution of Quality Indicator Data

In our metropolitan metropolis, I interviewed the quality control department of the department of health services. The goal of the interview was to learn how data for the various nursing-sensitive quality metrics are gathered and pooled. The proportion of people treated, the availability of healthcare personnel in an institution, and the quality of nursing care are all included in structural indicators, which examine the proportion of people treated, the availability of healthcare staff in an institution, and the quality of nursing care. The improvement of the patient’s condition or reaction to medicine supplied is used to assess the quality of care delivered by nurses to patients. However, patient outcomes are important.

How does the organization collect data on this quality indicator? 

Before measuring quality with a standardized tool, the quality assurance department collects information on structural indicators in a variety of methods. Concerning nursing staffing in a facility, the department gathers staffing data in each institution, as well as the average number of patients visited daily. It will aid in the development of a percentage to determine whether or not the institution has adequate nurse personnel., as well as the quality of care delivered by the nurses. The department also conducts patient surveys to collect data.

How does the organization disseminate aggregate data? 

After information collection, evaluation, and interpretation, the quality assurance department also provides feedback to the institution. Using excel spreadsheet and presentations, the department illustrates the result of many quality standards. The data gathered, it is processing, and the conclusions from the data obtained are all stated on the report returned to the facility. The report also contains a certification from the quality assurance department to the company regarding the nursing staff’s quality and number. When there are unskilled or non – certified nurses, the HR department jumps in and delivers summoning notices to the nursing staff, asking them to appear before the recruiting panel and defend the situation. Patient views on nurses are handled anonymously to avoid hostility among nurses and patients, but necessary criticism is offered to nurses based on the perspectives presented by the patients.

What role do nurses play in supporting accurate reporting and high-quality results?

Nursing plays an important role in ensuring accurate reporting and high-quality outcomes. Nursing actions are dependent on the available information, and a mistake in data collection can lead to prescription errors and other interventions that result in nursing care deaths. Nurses are in charge of capturing and entering patient information, verifying the data, and arranging it so that it makes sense to them and anybody else who may require access to the data. As a result, they are a critical component of the patient data found in facilities. The nursing record keeping codes state that the patient’s health care record must be an accurate characterization of care, plan of care, and execution, and also that the records must be taken as quickly as possible once an incident happens (Odom, 2021).

Importance of accurately entering data regarding nursing interventions

Proper recording and monitoring of all information demonstrate that the nurse has accepted responsibility for all clinical requirements. They have made every effort in hospital services to ensure that any carelessness or accidental error does not jeopardize the safety of patients. It is the nurses’ obligation to ensure that patients do not suffer as a result of any record keeping errors.; they should engage patients if necessary to ensure that the record is correct. It is very critical to provide healthcare services as it will save the lives of patients, but so is real record keeping because it can result in the loss of lives that the nurse is attempting to rescue. For example, if a nurse misidentifies high blood glucose levels as hypoglycemia, the patient is administered insulin, resulting in a continuing drop in blood sugar which may be deadly to the person. Paperwork is just as important as hands-on care; thus, nurses must make the obligation to continue to correct records for patient information.
