Caitlin’s parents, Janice and Doug, were informed of the diagnosis and a plan was developed to help the Bergen family care for their chronically ill daughter. The Bergens live in McKinley, an hour’s drive from Villa Hospital (Jong et al, 2019). Her parents were recently divorced, they both worked long hours, and the local doctors had little experience treating cystic fibrosis patients. When Caitlin returned to McHenry, Dr. Copeland, nursing staff, pulmonary technician Rebecca want to resume learning and treatment via telemedicine and videoconferencing. The Villa Health Hospital team will contact the Bergen family, the local pediatrician, Dr. Benjamin, local social workers, and Madeleine via phone and Skype to provide medical education and guidance on resources available in McHenry and to avoid unnecessary travel to Villa Health Hospital (Bird et al, 2019).

Evidence-Based Care Plan to Improve Safety and Outcomes

Now, I will develop an evidence-based care plan to enhance the patient’s safety and outcomes. The use of telemedicine as a form of telecom operation within multidisciplinary teams is recommended as an evidence-based treatment strategy to improve Caitlin’s safety and outcomes. Villa Hospital physicians, nurses, respiratory therapists, and social workers were able to discuss telehealth with Mr. and Mrs. Bergan, Dr. Benjamin, and Madeleine, a social worker from McHenry. According to the study, the word “telehealth” suggests to the application of technology for communication among patients and health care professionals. Evidence-Based Practice Model to Help Developing Care Plan

We will now propose strategies to address the challenges of interdisciplinary collaboration. The introduction of telemedicine has several benefits and is proven to save lives as well as improve outcomes in chronic disease management (Jong et al, 2019). At the village health center, Caitlin’s mother, Janice, Caitlin’s chest They were concerned about red patches on the skin near the area, and the use of Skype proved helpful. Through video conferencing, the Villa Hospital team was able to teach Janice the correct physical therapy position to avoid future redness. Bergan can use the Villa Hospital staff to provide telemedicine when Dr. Benjamin, the local paediatrician, is unavailable, especially on weekends and after hours (Dang et al, 2021). Dr. Benjamin and Madeline, a local social worker, also benefit from telemedicine and can work with hospital staff to resolve any issues related to Caitlin’s treatment. In this case, telemedicine issues could be the long working hours of Caitlin’s parents, the availability of resources in the remote village, and technical issues with Caitlin’s cell phone and internet connection. Local social workers can offer resources such as home visits, financial assistance to allow a parent to reduce their workload for children with specific health issues, and services to ensure adequate access to a cell phone and internet. According to the Agency for Healthcare Research and quality (AHRQ), telehealth has improved patient’s health and well-being and reduced mortality (2020). Telemedicine can minimize visits to health facilities, avoid unnecessary travel and costs, and treat chronic conditions at home (Jong et al, 2019).
