
Let me share some information with you about Caitlynn Bergan

Caitlynn is a two year old that recently presented to the emergency department with pneumonia. This is the second time within a 6 month time frame that she has been diagnosed with pneumonia. It has been noted that she had a meconium ileus at birth. She also presented with decreased breath sounds, rhonchi and fever. After further test Caitlynn has been diagnosed with cystic fibrosis.
Caitlynn’s parents, Doug and Janice Bergan, have been informed that Cystic Fibrosis is a chronic condition that will require ongoing healthcare for Caitlynn. The Bergan’s live in a very small town without access to resources they will need. The Bergan’s do have good health insurance but Mr. Bergan has recently been without work, so funds are pretty tight right now.

The Bergan’s live far from accessible health care and other resources. Caitlynn will be discharged from the hospital with home treatments in place. The Bergan’s will need extensive education on how to care for Caitlynn. Caitlynn will be discharged home with a physiotherapy vest, medication and breathing treatments and well as a specialized diet and supplements.
Now I will explain the ways in which an evidence-based practice model was used to help develop the plan of care for Caitlynn
We will consider the Knowledge-to-Action (KTA) EBP concept to evaluate problems other that travel and expense that could arise. We will need to assess the education needs of the Burgan’s. In order for them to care for Caitlynn appropriately they will be required to have an extensive knowledge of cystic fibrosis. Using this EBP we will find be able to help the family identify resources available to them. For example: credible online resources, also connecting them with a support group that they can meet with online can help them cope and learn. It will be care planned that the care team will meet with the Bergan’s once a week for 8 weeks or until they feel comfortable, them biweekly, then monthly. During these meetings we will have a Q&A where they will be encouraged to ask and answer questions so the care team can evaluate their understanding and promote a positive outcome for Caitlynn. A study that I found interesting from 2021, published by the International Journal of Environmental Research and Public Health, describes how chronic diseases are a major cause of death. With the advancements of telemedicine for people in remote or rural areas these diseases can be treated remotely, reducing the chances of decline. The modalities for the Bergan’s may include, chest physiotherapy, ongoing education that is accessible to them. Telehealth will provide the Bergan’s with access to health care professional without having to drive. Without close following of the healthcare team the risk for Caitlynn could be detrimental. For example, proper use of the physiotherapy vest, monitoring lung sounds and monitoring oxygen saturation along with bowel sounds via telehealth can help the Bergan’s avoid expensive trips to the hospital and can help decrease the chances of adverse effects from the disease process.
Next I will reflect on which evidence was most relevant and useful when making decisions regarding the care plan

I chose the journal articles listed for the criterion in the presentation to identify patient and family needs and help find solutions that will positively affect the outcomes. Positive outcomes for Caitlynn would consist of monitoring and treating issues before they resulted in hospitalizations or death. The decision to implement a telehealth program between the primary doctor, pediatrician, social workers and nurses was based on the necessity for continued health care. The Bergan’s inability to travel the long distance to the hospitals could cause increased harm to Caitlynn. Using evidence-based decision making will maximize the patients’ outcomes and decrease stressors to the family that is providing everyday care (Shepard et al., 2020)
Lastly, I would like to identify benefits and strategies to mitigate the challenges of interdisciplinary collaboration to care plan within the context of a remote team
The provider at the hospital was very proactive in initiating a remote collaboration with the Pediatrician. The interdisciplinary team was able to collaborate and identify the needs the Bergan’s would encounter in dealing with this chronic condition. The benefits of the interdisciplinary collaboration were that the Bergan’s could receive on-going and consistent support along with continuous education and follow up of their understanding, in their efforts to care for Caitlynn. The Bergan’s can have access to social workers, nurses, physicians and therapist without having to constantly travel long distances. The health care team and the Bergan’s are able to collaborate via telephone, skype texting and other forms to collaborate and support in the care of Caitlynn to achieve the best outcomes long term.
In conclusion, evidence-base practice models can enable the nurse to develop a person centered care plan that focuses on the safety and wellbeing of the patient and the caregivers, meeting their personal needs and wants, leading to enhanced patient outcomes (Fernandez et al., 2017).
