As Barker et al. (2020) and Reeves et al. (2019) postulated, interdisciplinary coordination can hardly be described as secondary given modern healthcare contexts; rather, it has become something that is crucial for enhancing client results as well as organizational efficiency. For a baccalaureate-prepared nurse, the concept of teamwork implies the ability to work with different care providers in delivering their services (AACN, 2020). However, the implemented change strategy adheres to the Plan-Do-Study-Act (PDSA) cycle which is similar to the nursing process regarding its cyclic nature and is widely employed in managing changes in healthcare (IHI, n. d. ).

This report has been undertaken based on an interview with one healthcare professional with a view to establishing an issue that is present in the healthcare system that could possibly be enhanced by inter professional collaboration. In accordance with a comprehensive plan of action to address recurring and novel issues affecting the healthcare field and its stakeholders, this report aims to create a foundation for conducting realistic and applicable interdisciplinary assessments in future evaluations. The Institute for Healthcare Improvement effectively demonstrates the application of the PDSA cycle, providing a structured approach to action-oriented learning and change implementation within the healthcare setting.

Interview Focused on Past or Current Issues

In the enlightening interview with Ms Sarah Johnson, a senior nurse manager at St. Mary’s Hospital, a comprehensive discussion of past and current challenges faced at the healthcare organisation was conducted, with a particular focus on patient safety and collaboration across disciplines (Johnson personal communication, April 10, 2021; Smith & Brown, 2019). Through a series of engaging activities such as open-ended questions, active listening, and probing for detailed aspects, examples, and reflections, invaluable insights were gleaned.

Some of the communication issues discussed by Ms Johnson include delays in the transfer of information where there were issues of handoffs, differences in the documentation of patient information, and failure to pass on important information. These breakdowns were considered as critical factors or adverse events that may harm the safety of the patients and the quality of care (Jones et al., 2020). furthermore, a concern that Ms. Johnson pointed out was medication errors such as those arising from medication reconciliation inconsistency during transitions of care (Anderson & Wilson, 2020). Patient harm was equally notable and she underlined that it is high time to improve the current processes in order to avoid such consequences.

Issue from an Interview

It is also important to support the thesis of a need for an evidence-based interdisciplinary approach to address the communication breakdowns at St. Mary’s Hospital for the following reasons. Secondly, interdisciplinary collaboration has been accredited by cost, effectiveness, and patient satisfaction, which enhances the effective care team model (Reeves, 2020).

St Mary’s Hospital can see the benefits of professional communication coaching across the staff, including improving effective communication between nurses, physicians, pharmacists, and other allied health professionals and Strengthening health care teams.

Potential Change Theories and Leadership Strategies

Leadership Strategies:

There are two change theories that can be used when seeking to understand and possibly apply interdisciplinary solutions for St. Mary’s Hospital and they are Kotter’s Eight-Step Model and Lewin Change Management Model. The conceptual framework that Kotter developed and described in his book titled “Leading Change” (2019) helps to break down the proper process for change.

Charting a course to improvement: The initial eight steps, starting with creating a sense of urgency and empowering action, can serve as a framework for the hospital for remediating communication breakdowns through interdisciplinary team activation/engagement and the development of a collective vision for improvement. On the same note, the Change Management Model was developed by Kurt Lewin, a social scientist, in 1951 and comprises three stages, namely, unfreezing, changing, and refreezing. When applied to St. Mary’s Hospital, this theory entails overturning the current norms of patterned communication, introducing interprofessional measures, and rewarding new patterns of communication-based on the promoted program.

Credible and Relevant Sources:

When it comes to Kotter’s Eight-step Model, there is a book by John Kotter called “Leading Change” published in 2019, which gives key details about the eight steps of the change process and also offers a practical approach on how it can be executed. As it has been discovered that it is being implemented across several sectors, including health, this source occupies a high level of reliability and usability for creating interprofessional strategies at St. Mary’s Hospital.

Thus, transformational leadership theory is discussed by Bass and Riggio in their book “Transformational Leadership” (2021), where they outline various aspects of the concept and its use in a broad range of organisations.

Collaborative Approaches From the Literature

According to the literature, there are many collaborative strategies that could provide insights into creating or developing interprofessional interaction to identify or resolve organizational challenges. There are various models addressing the concept of IPE, but the focus of this paper shall be on the Interprofessional Collaborative Practice (IPCP) model, which focuses on the articulation and sharing of the physical and mental endeavours of interprofessional students in achieving interprofessional aims of mitochondrial dysfunctions in patient care (Reeves et al. , 2019).

This model encourages universal, sisterly agreement and combined or collective understanding together with colleagues and friends, thus encouraging a good team spirit. The introduction of IPCP at St. Mary’s Hospital could benefit from improving collaborative practice and settling organizational concerns such as communication processes and patient care management amongst disciplines.

Credible and Relevant Sources:

To supplement the lack of literature on the case scenario and the Interprofessional Collaborative Practice model, Reeves et al. ’s article titled “Interprofessional Collaboration to Improve Professional Practice and Healthcare Outcomes” (2019) offers a wealth of information on the principles of IPCP and its implementation in healthcare contexts.

This source was deemed highly relevant and credible for addressing St. Mary’s Hospital’s paradox because it elucidated the formation of interdisciplinary teams and the resolution of Relative to the TeamSTEPPS framework, the Agency for Healthcare Research and Quality (AHRQ) provides reliable and pertinent information, encompassing workshops for the training, guides for implementation, and research data, to advance the application of TeamSTEPPS in healthcare systems (AHRQ, 2019). The following resources will, therefore, offer more tangible tips on how to foster effective teamwork and communication within interdisciplinary teams in line with the challenges faced by St. Mary’s Hospital.

Conclusion

Interviewing Ms Sarah Johnson, a senior nurse manager at St. Mary’s Hospital, has gathered important information pertaining to various concerns that cannot be overlooked within the organisation, especially those that relate to a lack of effective communication and patient safety concerns. Moving back to the past and some modern problems of oncology, it becomes clear that interdisciplinary cooperation is one of the most important elements in resolving these problems.

Drawing from the current knowledge and effective practice features as outlined in the literature, St. Mary’s Hospital can establish cross-functional interventions that enable the organisation to care for patients cooperatively, embrace a teamwork approach, and yield better results. The above-discussed issues throw a lot of light on the importance of being more active in exercising changes that might help in improving the overall organisational performance. Traditional and modern theories, concepts and models of change include Kotter’s 8-Step Model and Lewin’s Change Management Model, while leadership approaches include transformational leadership and shared leadership at St. Mary’s Hospital can help the hospital offer effective change and leadership to various interdisciplinary programs.
