The cooperation between various kinds of healthcare specialists enhances the communication process, reduces the waiting times, and provides the patient with improved outcomes. To ensure the lasting success of the interdisciplinary collaboration process, teams should undertake measures to develop teamwork, ensure that all are on the same level, and care is not lost in transferring patients among the providers. This evaluation will be based on the practical techniques and tools that would enable the healthcare teams to interact efficiently, reduce time lag in patient care, and improve the quality of healthcare.
Interview Summary
As I was going through a case during the interview, I overheard that it may be problematic to coordinate care handover across various departments within a given hospital and external facilities (Khatri et al., 2023). The interviewed individual in this position will coordinate the patients, manage the nursing staff, and maintain appropriate communication among the staff. The charge nurse claimed that the failure to communicate appropriately leads to delayed response to medical cases, taking unnecessary actions, and dissatisfaction of patients. The organization also implemented Electronic Health Records (EHRs) and held training on communication, but those attempts were undermined by the staff not adhering to the new procedures properly and a lack of supervision after that.
Riverstone Medical Center emphasizes that teamwork is a requirement, yet it is hard to make it work on a day-to-day basis. Leaders assist teams in communication with each other, and teamwork tends to decrease between meetings and ordinary activities (Henson et al., 2020). They explained how collaboration enables healthcare experts and case managers to develop personalized modalities of assisting patients with multiple health problems. However, these efforts are not connected with each other to create a single system. It was also evident in the interview that care coordination has significant challenges and that the introduction of organized team-based approaches would contribute to the process and the outcomes of the care.

My interview with the nurse at Riverstone Medical Center revealed that the process of coordinating the care transition, particularly the one between departments or to an external provider, is a very challenging task. The issues in this situation lead to difficulties in communication and the late provision of medical assistance. Healthcare professionals should cooperate and coordinate efforts to guarantee that the patients are provided with uninterrupted care. The collaboration will help the team to make sure that medical and rehabilitative needs are not taken into consideration in separate care plans, eliminating gaps in care (Phillips et al., 2020). The improvement of patient care during transitions has led to better coordination, which includes the avoidance of mistakes due to communication and improved functioning in the team and resource savings.
