Health Care Delivery Model and Nursing Practice

A professional practice model is the overall framework under which nurses provide care. It consists of the care delivery model (structure and procedures), cooperation (relationships), and attitudes that enable nurses to contribute to both patient and environmental outcomes. In an ideal world, nursing care delivery models would match the number and kind of caregivers to the patient care needs, establishing who would do what responsibilities, who is responsible, and who makes decisions, as well as specifying assignments, responsibility, and authorization to provide patient care.

American Rescue Plan Act

President Biden signed the American Rescue Plan Act (ARPA) into law on March 11, 2021. The $1.9 trillion bill is designed to give far-reaching relief from the ongoing COVID-19 pandemic’s economic and other consequences (American Rescue Plan Act, 2021). The ARPA provides comprehensive assistance that might have a direct impact on your financially, in addition to money for testing, contact tracing, vaccines, education, and state and local governments. Under the ARPA, many people will receive a third round of direct payments (which the law calls recovery rebates). It provides for direct payments of $1,400 — plus $1,400 per dependent — for single tax filers with adjusted gross income (AGI) up to $75,000 per year, heads of households with AGI up to $112,500 and married couples with AGI up to $150,000. The rebates phase out when AGI exceeds $80,000, $120,000 and $160,000, respectively. Dependents include adult dependents, such as college students and qualifying family members (American Rescue Plan Act , 2021).

Furthermore, as a result of the Biden-Harris Administration’s Special Enrolment Period for the COVID-19 Public Health Emergency, consumers can continue to apply for coverage, compare plan options, and learn if they qualify for lower premiums by visiting HealthCare.gov or CuidadoDeSalud.gov until August 15. Customers may examine 2021 plans and pricing and apply to determine whether they are qualified for enrolment and financial aid. They can enrol in a plan that best suits their needs if they are qualified. To obtain the additional savings under the ARP, current enrolees should check their application, make any necessary adjustments to their current information, submit their application, and then choose a new plan or reselect their current plan (Summary of American Rescue Plan Act of 2021 and Provisions Affecting Hospitals and Health Systems | AHA, 2021).

The ARP Act will assist in aligning the nursing workforce with current developments in the field. Nurses are given financial assistance to further their education, as well as assistance in rural and underprivileged regions. It is the nurse’s obligation to uphold the professional standards set for nurses and to follow any rules that are enforced. The law allocates $450 million to assist skilled nurses in protecting themselves against COVID-19; $200 million to develop and disseminate COVID-19 prevention protocols in collaboration with quality improvement organizations; and $250 million to states and territories to deploy strike teams to assist SNFs experiencing COVID-19 outbreaks.

Nurses around the country have praised the APR legislation. It advocates for an Occupational Health & Safety Administration-enforced Emergency Temporary Standard to safeguard nurses and other significant healthcare personnel. The new law provided nurses newfound faith that their demands will indeed be acknowledged. President Biden’s strategy will permit for the manufacture and supply of PPE and other necessary medical supplies, test kits, and vaccines in the necessary quantities to safeguard staffs and the public at large.

Quality Measures and Pay for Performance

Pay-for-performance (P4P) has been proposed as a potential option for cost reduction as well as quality enhancement. P4P is a financial incentive-based method for improving healthcare quality. P4P aims to promote adherence to best practices by offering financial incentives to healthcare providers. Adherence to clinical guidelines is considered to enhance clinical outcomes and quality of care. Adherence to optimal clinical recommendations and practice, whether right or wrong, is not always as great as may be expected (Freeman et al., 2017).

Higher data revealed that P4P had a balanced effect on patient health outcomes and a favourable influence on reducing re – admissions rates in the hospital environment. Even though many studies discovered beneficial effects associated with P4P programs, the results have been conflicting across research, the amplitude of effect was frequently limited, even though it was challenging to convincingly relate observed differences in consequences to the treatment itself due to the observational nature of most research and their various evaluation shortcomings (Freeman et al., 2017). This results in improved recovery as a result of enhanced evidence-based therapy, which contributes to effective and safe treatment options and a reduction in inpatient admissions. It does have an impact on nurses’ jobs since it demands them to give patient-centred evidence-based care as well as the creation of quality standards and ethical guidelines.

The ARP Act, passed in response to the COVID-19 pandemic, offers assistance to governments, including tribal governments like the Band, including through the formation of the FRF, to offset the budgetary consequences of the Emergency. Payment for performance had no influence on patient health outcomes but had a positive effect on reducing hospital readmissions in the hospital setting. The use of nursing sensitive quality metrics can aid the industry’s push toward evidence-based practice and value-based payment (Kyeremanteng et al., 2019). Structure measurements can assist hospitals in maximizing the efficacy of their existing systems and facilities in order to deliver the best possible care to their patient populations.

Leadership and Management Roles Importance

The ARP Act addresses the need of focusing on health promotion and prevention. It also focuses on the importance of leadership and management In nursing profession The duties of nursing professionals who guide nursing personnel, supervise the organizational structure of medical procedures, and lead nursing teams in providing patient care are referred to as nursing leadership and management. When the entire team succeeds, nursing leadership and management are most successful. This nurse leadership style has been linked to improved patient outcomes, less medical errors, and increased staff retention, all of which benefit the healthcare workforce as a whole. Nursing leaders must be aware of nursing trends and how they may affect the future of health care, whether the shift is in patient demographics or health care delivery. This awareness may allow them to have a positive impact on care delivery (USAHS, 2020).

Nursing professional management and leadership coming from today ’s healthcare advancements to enhance safety and quality of patients. One of them is stepping over management.  Nursing education has improved professional nurses’ leadership skills, ability to manage patients, colleagues, as well as other sectors in healthcare systems. The approach is established to encourage patient care and safety. The nurses need to learn how to construct treatment strategies utilizing data obtained digitally in order to plan and develop evidence-based treatment. Ensuring patient safety is another role played by nurses. They play a key role of addressing modern healthcare practices based on enhanced and better intervention. Patient empowerment, as an ethical quandary, requires practitioners to enable patients in making choices or even provide authorization on topics pertaining to their health and safety. Nurses must ensure a secure environment and discuss first before developing life decisions about the medical illness in the health sector.

Communication with patients and co-workers is also essential in nurse management. Currently, nursing practice is multidisciplinary, nurses and other clinicians all working toward the same objective. The use of clinical visits and referring to other physicians for check-ups is encouraged for nursing professionals in order to establish performance and improving quality of treatment.

Predictions for practice of nursing and nursing roles transformation within the next 5 years

The nursing profession will evolve dramatically in the next years. In the nursing business, there will be more training programs, better hospitals, greater responsibility, a sense of family, and a focus on patient care, which will save lives and produced generations of committed medical professionals. (The Future of Nursing | Global Health Education, 2020) As the scope of health care will evolve and physician shortages worsen, nurses will see changes such as greater specialization and nurses migrating into community outpatient settings. According to research, the need for registered nurses specialized in certain disciplines of medicine such as gerontology and psychiatry, among others, is increasing. There are more training programs, better hospitals, ultimate responsibility, a sense of community, as well as an emphasis on patients regarding the nursing industry, all of which have saved many lives and generated thousands of devoted health personnel.

Increased Telemedicine Popularity and Data Protection: As a result of the epidemic, the usage of telemedicine services increased significantly in 2020, creating privacy and security issues for all healthcare practitioners. Nurses who provide care via telehealth must be conversant with full consent and HIPAA rules. Every nurse should obtain the patient’s permission before using telemedicine technology. That authorization should specify the procedures to be followed in the event of a technological breakdown during the consultation (Vermeulen et al., 2019).

Emergence of the Nurse Navigator: Nurse navigators are nursing staff who use their medical training as well as their administrative and interpersonal abilities to assist patients throughout managing their health-care journeys. They help individuals understand the healthcare systems and explore integrative approaches to improving their overall quality of life. A nurse navigator’s duty is analogous to that of a nursing patient advocate (Vermeulen et al., 2019). The navigator aids the individual in understanding the system and receiving care. They assist patients with understanding what is happening on with them, including testing, diagnoses, medications, and all other forms of treatment. As the population ages and chronic health issues grow more common, this position will become increasingly important. Nursing navigators might work for themselves, for an independent nursing agency, for an insurance company, for physician groups, or for medical institutions (Vermeulen et al., 2019).

Conclusion

The APR act has a huge effect on healthcare sector of US and it will increase further due to COVID-19 situation which is worsening throughout the year. This act has shaped many leadership and management roles in nursing profession and changed the very basic model of payments in the nursing profession, It also helps them financially to increase theory efficiency and gives them incentives for their better work management, The need for registered nurses who specialize in certain disciplines of medicine is rising. The trends are anticipated to continue affecting how the nursing profession and practice evolve in the future. This act also deals with the predictions for the next 5 years and helps reinforcing the health care system and aids them in empowering it and tackle the challenges they will face in the upcoming years.
